Gynecomastia

Gynaecomastia is a benign enlargement of the glandular breast tissue in males. It can appear in newborns, during puberty and in older men, and is often due to a shift in the balance of oestrogen and testosterone.

There may be a firm, rubbery disc of tissue beneath the nipple on one or both sides, sometimes tender. Examination separates true gynaecomastia from fat (pseudogynaecomastia), and tests of liver, kidney and hormone levels look for causes such as medicines, anabolic steroids, cannabis, liver disease or, rarely, tumours. Ultrasound or mammography can exclude breast cancer if a lump is hard or irregular.

Many cases, especially in puberty, settle within a year or two. Stopping a causative drug or treating an underlying condition may help. If the breast tissue persists or causes distress, surgery is the effective option: liposuction for fatty enlargement and excision of the gland for firm tissue, often combined. Recovery takes a few weeks with a compression garment. Some people travel abroad for cosmetic surgery.

  • A clinical examination note distinguishing glandular breast tissue from fat, with measurements and any tenderness.
  • Hormone and blood tests usually requested: testosterone, oestradiol, LH, prolactin, liver, kidney and thyroid function.
  • A list of medicines and substances you use, since some drugs, anabolic steroids and cannabis can cause gynecomastia.
  • Breast ultrasound or mammogram reports if there is a hard, one-sided or fixed lump.
  • How long you have had the enlargement, since long-standing tissue becomes fibrous and is less likely to regress.

  • There is a hard, fixed, irregular or bloody lump or nipple discharge, which needs checking for male breast cancer.
  • The enlargement began suddenly or you have symptoms of a hormone or testicular problem.
  • Surgery is advised before your hormones and medicines have been reviewed.
  • You are a teenager and surgery is proposed within the first year or two of onset.

  1. Is this true glandular tissue or mainly fat?
  2. Could a medicine or hormone problem be causing it, and can it be corrected?
  3. Is any treatment needed, or is it likely to settle by itself?
  4. Would you use liposuction, gland excision or both?
  5. What are the risks of scarring, uneven contour or fluid collection?
  6. Will it recur if the cause continues?

Many cases, especially in adolescents and those caused by a medicine, regress over months to a couple of years. Tissue present for more than about a year tends to be fibrous and persists. Surgery, which combines liposuction with gland removal, gives a lasting flatter chest in most patients. Complications are usually minor, such as bruising, contour irregularity or fluid collection. Recurrence is possible if weight gain or the causative factor continues.

Will it go away by itself?

Often yes, especially in teenagers or when a medicine is the cause, over months to two years. Long-standing tissue is less likely to shrink.

Is gynecomastia dangerous?

It is almost always benign, but a hard, fixed or one-sided lump, or nipple discharge, needs examination to exclude male breast cancer.

What does surgery involve?

Typically liposuction for fatty tissue, with gland excision through a small cut at the areola edge for firm glandular tissue. It is usually a day procedure.

Clinical context

Working up gynecomastia properly

A condition like gynecomastia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a सौंदर्य शल्य चिकित्सा specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
