Gummy Smile

A gummy smile is a smile in which an excessive amount of gum shows above the upper teeth, usually more than 3 to 4 mm. It may be caused by teeth that look short, an overactive lip or a long upper jaw.

It is mainly an aesthetic concern rather than an illness. A dentist or periodontist assesses the cause by examining the teeth, gum levels and lip movement, with X-rays if needed. Causes include altered passive eruption, where the gum covers too much of the tooth, a hypermobile upper lip, or vertical overgrowth of the upper jaw. It does not worsen by itself.

The treatment depends on the cause. Gum contouring, by scalpel or laser, removes excess gum and reshapes the margin and is suitable when the teeth are short. Botulinum toxin can reduce lip movement temporarily, lip repositioning surgery is a longer-lasting option, and orthodontics or jaw surgery treats skeletal causes. Combined care by a cosmetic dentist is common, and some people choose treatment abroad.

  • Dental X-rays and clinical photographs at rest and smiling, so the cause of the gum display can be judged.
  • A periodontal assessment of gum health, pocket depths and the position of bone relative to the tooth surface.
  • Measurements of lip length and mobility, and tooth proportions, from your dentist.
  • A cephalometric X-ray or 3D scan if a jaw-related cause is suspected.
  • Your dental history, including orthodontics, gum disease and medicines such as calcium channel blockers or phenytoin that cause gum overgrowth.

  • Gum contouring is advised but no one has checked whether the cause is jaw position or lip movement.
  • You are offered jaw surgery or lip repositioning and want to compare it with less invasive options.
  • You have active gum disease or gum overgrowth from medicine, which needs treating first.
  • A previous gum procedure has relapsed or left uneven gum levels.

  1. What is causing my gummy smile: gum, tooth length, jaw or lip?
  2. Is simple gum contouring enough, or do I need bone reshaping, lip surgery or botulinum toxin?
  3. How far will the gum edge move, and will it look natural?
  4. Is my gum disease controlled before any treatment?
  5. What is the chance of the gum growing back?
  6. How will the result be planned and previewed?

Results are generally good when the treatment matches the cause. Gum contouring works where excess gum covers normal-length teeth and is permanent in most people, though gum can regrow if the bone level was not addressed. Jaw-related gummy smiles need orthognathic surgery for a lasting result, and botulinum toxin is temporary. Healing takes a few weeks, with careful oral hygiene needed to protect the gum.

Is gum contouring permanent?

Usually yes, if the cause is excess gum. If the underlying bone sits too high, gum can drift back, so bone reshaping may be needed.

Can botulinum toxin fix a gummy smile?

It can reduce gum show caused by an overactive upper lip, but the effect wears off after a few months and needs repeating.

When is jaw surgery needed?

When the upper jaw is vertically too long, which is judged on a profile X-ray, correction of the jaw gives the lasting result.

Clinical context

Working up gummy smile properly

A condition like gummy smile is rarely well-served by jumping straight to treatment shopping. The sequence below is what a दंत चिकित्सा specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or दंत चिकित्सा specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
