Many people have no symptoms. Others faint or suffer sudden cardiac arrest, typically at rest, during sleep or with fever. Diagnosis rests on a characteristic ECG pattern, sometimes revealed by a drug challenge, and genetic testing and family screening. The risk of cardiac arrest varies, and doctors weigh previous fainting, a spontaneous ECG pattern and family history in judging it.

Everyone with Brugada syndrome is advised to treat fever promptly and avoid triggering drugs and excess alcohol. An implantable cardioverter defibrillator (ICD) is the proven protection for those who have survived a cardiac arrest or have unexplained fainting and a high-risk pattern. Quinidine or catheter ablation may help people with repeated shocks. Relatives are offered testing, and expert electrophysiology centres guide the decision.

  • ECG recordings showing the Brugada pattern, with any drug challenge test (such as ajmaline) result.
  • A history of fainting, seizures, nocturnal gasping, palpitations or cardiac arrest, with dates and circumstances.
  • Family history of sudden unexplained death at a young age or of known Brugada syndrome.
  • Genetic test results, including SCN5A, if performed, and results from relatives.
  • Echocardiogram, Holter monitoring and any electrophysiology study report.

  • A defibrillator has been recommended for a person who has never fainted and has no family history of sudden death.
  • You have been diagnosed solely from an ECG pattern without a confirming challenge test.
  • You have had unexplained fainting and defibrillator implantation has been delayed or declined.
  • Frequent arrhythmias are occurring and ablation has not been discussed.

  1. Do I have type 1 Brugada pattern, and has it been confirmed properly?
  2. What is my individual risk of dangerous arrhythmia?
  3. Do I need a defibrillator, and what factors decide that?
  4. Which medicines and situations should I avoid, and how do I manage fever?
  5. Should my family members be screened?
  6. Could catheter ablation help me?

Risk varies greatly. People who have survived cardiac arrest or had arrhythmic fainting have the highest risk of recurrence and generally benefit from a defibrillator. Those with no symptoms and no family history usually have a much lower risk, and management centres on avoiding triggers such as fever and certain drugs. A defibrillator can prevent sudden death but can cause inappropriate shocks and device complications. Regular specialist follow-up allows risk to be reassessed.

Will everyone with Brugada syndrome need a defibrillator?

No. It is advised mainly for those who have had cardiac arrest or serious arrhythmic symptoms; others are assessed individually.

What should I do when I have a fever?

Treat it promptly with paracetamol, since fever can trigger arrhythmias, and seek medical advice.

Can my children inherit it?

It is usually inherited as a dominant trait, so each child has a 50% chance if a gene variant is present.

Can I fly with a defibrillator?

Yes, with your device card; discuss recent shocks with your doctor.

Clinical context

Working up brugada syndrome properly

A condition like brugada syndrome is rarely well-served by jumping straight to treatment shopping. The sequence below is what a हृदय शल्य चिकित्सा specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
