A baby usually appears healthy at birth, then develops yellow skin and eyes beyond two weeks of age, with pale stools and dark urine. Blood tests, ultrasound and a liver biopsy support the diagnosis, which is confirmed at surgery. Without treatment, scarring of the liver (cirrhosis) develops within months, so early diagnosis is critical and affects how well the liver survives.

The first operation is the Kasai procedure, in which the blocked ducts are replaced with a loop of bowel to allow bile to drain. It works best when done in the first two months of life. Many children still develop liver failure later, and then need a liver transplant, often using a part-liver from a living donor, such as a parent. Care is lifelong, and specialist paediatric liver units are essential.

  • Serial blood tests showing total and conjugated bilirubin, liver enzymes, albumin and clotting, with dates so the trend is visible.
  • Abdominal ultrasound report, including gallbladder appearance and any triangular cord sign, and any HIDA scan or liver biopsy report.
  • The operative note and date of the Kasai portoenterostomy if it has been done, with the child's age at surgery.
  • A record of cholangitis episodes, weight gain, vitamin supplements, bleeding problems and ascites.
  • Blood group, vaccination record and parents' details if a living-donor liver graft is considered.

  • Your baby is over about eight weeks old with jaundice and the Kasai operation has not been discussed, because timing strongly influences the result.
  • Jaundice has not cleared after Kasai, or has returned, and nobody has assessed the child for transplant.
  • Repeated cholangitis or signs of portal hypertension such as a large spleen or bleeding have developed.
  • A living-donor liver transplant is being considered and you want the donor assessment and risks explained.

  1. Has biliary atresia been proven, and what other causes of jaundice have been excluded?
  2. Is the Kasai operation still appropriate at my child's age, and where is it best done?
  3. How will we know whether the Kasai has worked?
  4. What signs of cholangitis or liver failure should prompt immediate care?
  5. When should we start planning a liver transplant, and what are the donor options?
  6. How should my child's feeding, vitamins and vaccinations be managed?

Without surgery biliary atresia is fatal in early childhood. The Kasai operation works best when done in the first two months of life; about half of children achieve good bile flow and clear jaundice, and some of these do well for years. Most children, including many after a successful Kasai, eventually develop liver scarring and need a transplant. Transplant outcomes in children are good, with long-term survival in the large majority. Outcome depends on age at surgery, bile flow, and complications such as cholangitis.

How quickly must the Kasai operation be done?

As early as possible. Results are best when it is done in the first two months of life, and outcomes decline with later surgery, though it may still be considered.

Will my child need a liver transplant?

Many children do eventually, even after a successful Kasai. Some live for many years with their own liver. Regular monitoring shows when transplant planning should start.

Can a parent be a liver donor?

Often yes. A healthy, compatible adult can donate part of the liver, which regrows. Donors undergo thorough assessment first.

Is it caused by something the parents did?

No. The cause is not fully understood and is not due to anything done in pregnancy or after birth.

Clinical context

Working up biliary atresia (infant) properly

A condition like biliary atresia (infant) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a अंग प्रत्यारोपण specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or अंग प्रत्यारोपण specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
