Angle-Closure Glaucoma

Angle-closure glaucoma happens when the drainage angle of the eye becomes blocked, so eye pressure rises and can damage the optic nerve. It is more common in older people and in those who are long-sighted.

An acute attack causes severe eye pain, a red eye, blurred vision, halos around lights, headache and vomiting, and needs emergency care. Chronic forms can be silent until vision is lost. Diagnosis uses pressure measurement, gonioscopy to view the angle, and optic nerve and visual field tests.

An acute attack is treated urgently with pressure-lowering drops, tablets or drips, followed by laser peripheral iridotomy to create a drainage channel. Cataract surgery, which deepens the angle, is often advised too. If pressure stays high, drainage surgery such as trabeculectomy or a glaucoma drainage valve (Ahmed) may be needed. Lost vision cannot be restored, so early treatment is vital.

  • Eye examination report with intraocular pressure readings and gonioscopy findings describing the drainage angle.
  • Optical coherence tomography (OCT) of the optic nerve and visual field test results.
  • Anterior segment OCT or ultrasound biomicroscopy images, if available.
  • Records of laser iridotomy or other procedures, and medicines used to lower eye pressure.
  • A history of any acute attack with pain, halos, redness or nausea, with dates.

  • You have had an acute attack and want advice on preventing damage to the other eye.
  • Pressure remains high after laser iridotomy and you are comparing lens extraction with trabeculectomy or a drainage valve.
  • Visual field loss is progressing despite treatment.
  • You have narrow angles with no symptoms and are unsure whether treatment is needed.

  1. What optic nerve damage and visual field loss do I have now?
  2. Will laser iridotomy be enough, or do I need lens surgery or pressure-lowering surgery?
  3. Does the other eye need preventive laser?
  4. What are the warning signs of another attack that need emergency care?
  5. Which eye drops should I avoid, and which must I continue?
  6. How often will pressure and visual fields be checked?

Vision lost to glaucoma cannot be restored, but treatment can stop further damage. Prompt laser iridotomy or lens extraction opens the drainage angle and prevents attacks in most people. Some still need long-term eye drops or drainage surgery. Outcome depends on the optic nerve damage present at diagnosis, how quickly an attack is treated, and regular lifelong monitoring.

Is angle-closure glaucoma an emergency?

An acute attack is an emergency with severe eye pain, blurred vision, halos and vomiting. It needs treatment within hours to protect vision.

Will laser cure it?

Laser iridotomy opens a path for fluid and prevents attacks in many people, but some still need drops or surgery to keep pressure low.

Can I fly with narrow angles?

Flying is generally safe after treatment. If you have untreated narrow angles and symptoms, ask your eye doctor first, and seek help urgently for eye pain.

Clinical context

Working up angle-closure glaucoma properly

A condition like angle-closure glaucoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a नेत्र विज्ञान specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or नेत्र विज्ञान specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
