Achalasia

Achalasia is a rare swallowing disorder in which the muscle at the lower end of the gullet fails to relax and the oesophagus loses its normal squeezing movement. It usually appears in adulthood.

People notice food sticking after swallowing, regurgitation of undigested food, chest pain, night-time coughing and gradual weight loss. It is often mistaken for reflux at first. Diagnosis rests on a barium swallow, upper endoscopy to exclude other causes, and oesophageal manometry. The condition is long-term and slowly progressive, but symptoms can be controlled.

Treatment aims to open the lower sphincter, because no tablet restores normal oesophageal movement. Options include pneumatic balloon dilatation, laparoscopic Heller myotomy and peroral endoscopic myotomy (POEM), chosen by manometry pattern, age and fitness. Botulinum toxin injection is a short-term option for frail patients. People travel abroad mainly to reach units experienced in POEM or myotomy.

  • Barium swallow (timed barium oesophagram) images showing the narrowed 'bird's beak' outlet and any oesophageal dilatation.
  • Oesophageal high-resolution manometry report, which confirms the diagnosis and the achalasia subtype (I, II or III).
  • Upper gastrointestinal endoscopy report and biopsies, usually requested to exclude cancer or a stricture mimicking achalasia.
  • Records of any previous balloon dilatation, botulinum toxin injection or surgery, with dates and how long symptoms improved.
  • Weight history and recent blood tests, since long-standing swallowing difficulty can cause weight loss and poor nutrition.

  • You were diagnosed on symptoms or endoscopy alone, without manometry, as other conditions can look similar and treatment depends on the exact subtype.
  • Symptoms have returned after a previous dilatation or myotomy and you are being offered a repeat procedure.
  • You are told surgery and POEM are equivalent without discussion of reflux risk after each option.
  • Your weight is falling or you regurgitate food at night with coughing, which suggests more advanced disease.

  1. Which achalasia subtype do I have on manometry, and does it change the choice between POEM, surgical myotomy and dilatation?
  2. How likely is acid reflux after each procedure, and how would it be monitored and treated?
  3. Has cancer been properly excluded as the cause of my swallowing difficulty?
  4. What happens to my oesophagus over the years if I choose no procedure?
  5. How often will I need follow-up endoscopy or barium studies after treatment?
  6. If the first treatment wears off, which options remain open to me?

Achalasia cannot be cured, but swallowing improves in most people after myotomy, whether performed surgically or endoscopically, or after pneumatic dilatation. Relief is usually good in the first years, although some people need repeat treatment. Reflux is a common later issue, and lifelong follow-up is advised. Outcomes depend on subtype, how dilated the oesophagus is before treatment, and prior treatments.

Can achalasia be cured?

The underlying nerve problem cannot be reversed, but swallowing can be restored for years by cutting or stretching the tight sphincter. Some people need repeat treatment, and long-term follow-up is advised.

Will I get acid reflux after treatment?

Reflux is a recognised side effect of myotomy because the valve is weakened. It is usually controlled with acid-suppressing medicines, and surveillance may be advised for some people.

Is it safe to fly with achalasia?

Flying is generally safe, but regurgitation and choking can occur when lying back. Eat slowly, avoid heavy meals before and during the flight, and carry your medical records.

Clinical context

Working up achalasia properly

A condition like achalasia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a जीआई सर्जरी specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or जीआई सर्जरी specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
