Urinary Retention (BPH-related)

Urinary retention from benign prostatic hyperplasia happens when an enlarged prostate blocks the flow of urine and the bladder cannot empty. It affects older men and can come on suddenly or develop gradually.

Acute retention causes painful inability to pass urine and needs urgent catheterisation. Chronic retention may cause a weak stream, frequent small voids, dribbling and infections. Diagnosis involves examination, ultrasound for residual urine, a prostate blood test and kidney function checks. Without treatment, repeated retention can harm the bladder and kidneys.

A catheter relieves the first episode, often with alpha-blocker tablets and sometimes a finasteride-type medicine, then a trial without catheter. If retention returns or medicines fail, surgery is advised: transurethral resection of the prostate or holmium laser enucleation, the latter suited to larger glands. Travelling abroad is usually considered once the situation is stable.

  • Prostate ultrasound or MRI reports with prostate volume, and a bladder ultrasound showing residual urine.
  • PSA blood test results, which doctors usually request to exclude prostate cancer.
  • Kidney function tests and a kidney ultrasound to check for pressure effects from the blocked bladder.
  • Urodynamics or uroflowmetry, if available, and a record of catheter use including start date and any failed trial without catheter.
  • A full medicine list, especially blood thinners and prostate drugs such as alpha-blockers and 5-alpha reductase inhibitors.

  • You have been told to keep a long-term catheter without discussing whether surgery could remove the need for it.
  • Your PSA is raised or your prostate feels abnormal and cancer has not been properly excluded.
  • You have a very large prostate and are advised to have an operation that may not suit that size.
  • You have heart disease or take blood thinners and are unsure which procedure is safest.

  1. How large is my prostate, and has cancer been ruled out?
  2. Is my retention from the prostate alone, or could bladder weakness be involved?
  3. Which procedure suits my prostate size and health best, HoLEP, TURP or another?
  4. What are the risks to ejaculation, continence and erections?
  5. How likely is it that I will be able to pass urine normally and remove the catheter?
  6. What happens if I do not have surgery and stay on a catheter?

Most men with retention caused by BPH who have surgery can pass urine again and no longer need a catheter. HoLEP and TURP both give lasting relief, and HoLEP tends to be effective across a range of prostate sizes. Success is lower when the bladder muscle has been weakened by long-standing retention. Common effects include temporary urgency or leakage and dry ejaculation. Untreated retention risks infection, bladder stones and kidney damage.

Will I need a catheter for life?

Not necessarily. Many men can have surgery to relieve the blockage and then pass urine normally. Some, whose bladder muscle has become weak, may still need a catheter or self-catheterisation.

What is the difference between HoLEP and TURP?

Both remove obstructing prostate tissue through the urethra. HoLEP uses a laser to remove the tissue in larger pieces and can treat large prostates, often with less bleeding. TURP uses an electrical loop and is widely available.

Will surgery affect sex?

Dry ejaculation is common, as semen passes backwards into the bladder. Erections are usually not affected much, but you should discuss your concerns with your surgeon.

Can I fly with a catheter?

Yes, with a leg bag and some planning, but check with your doctor first. Blockage, infection or bleeding can occur and travel is best after the catheter plan is clear.

Clinical context

Working up urinary retention (bph-related) properly

A condition like urinary retention (bph-related) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a المسالك البولية specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or المسالك البولية specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
