عن uterine fibroids (symptomatic)

ما تحتاج معرفته

Common symptoms are heavy or prolonged periods, anaemia, pelvic pain, frequent urination and a bloated abdomen. Pelvic ultrasound shows the fibroids, and MRI maps their size and position. Many are found by chance and need no treatment, and they usually shrink after menopause.

Medicines such as tranexamic acid, hormonal treatments and the hormone-blocking drug ulipristal or GnRH analogues ease bleeding or shrink fibroids for a time. Procedures include uterine fibroid embolisation, and hysteroscopic, laparoscopic, robotic or open myomectomy, which remove fibroids while keeping the womb. Hysterectomy is definitive when childbearing is complete. Women often travel abroad for myomectomy or embolisation when womb-preserving options are limited locally.

الفحوصات قبل السفر

فحوصات يجب إجراؤها مسبقاً

  • Pelvic ultrasound or MRI showing the number, size and position of fibroids, especially whether they bulge into the womb cavity.
  • Haemoglobin and iron studies, because heavy bleeding often causes anaemia.
  • A menstrual diary and a record of medicines already tried, such as tranexamic acid, hormonal treatments or an intrauterine device.
  • A recent cervical screening result and, if bleeding is irregular, an endometrial assessment.
  • Your fertility plans and any history of miscarriage or earlier gynaecological surgery.

رأي ثانٍ

متى تطلب رأياً آخر

  • Hysterectomy has been advised and you wish to keep your womb or fertility.
  • You have heavy bleeding with low haemoglobin and have not tried medical treatment.
  • A fibroid is growing quickly or after menopause, which needs careful review.
  • You are choosing between myomectomy and embolisation and want to know the effect on future pregnancy.

أسئلة يجب طرحها

  1. How many fibroids do I have, and where are they, and are they causing my symptoms?
  2. Do I have anaemia that needs treatment first?
  3. Which option suits me best: medicines, myomectomy, embolisation or hysterectomy?
  4. How would each choice affect my chances of pregnancy?
  5. Could fibroids return after treatment, and how likely is that?
  6. How long is recovery with each option?

التوقعات

تشخيص واقعي

Fibroids are benign and become less troublesome after menopause, so some women need only monitoring. Treatment relieves heavy bleeding and pressure in most women. Myomectomy preserves the womb, but new fibroids grow back in a good proportion of women over time, and embolisation avoids major surgery but may suit fertility plans less well. Hysterectomy is the only definitive cure. Outcomes depend on fibroid size, number and position.

04 · الأسئلة الشائعة

أسئلة شائعة حول uterine fibroids (symptomatic)

Do fibroids always need treatment?

No. Fibroids that cause no problems can be watched, and they often shrink after menopause. Treatment is for heavy bleeding, pain, pressure or fertility problems.

Can fibroids come back after myomectomy?

Yes. New fibroids develop in a fair proportion of women over the years, particularly if you had several. A hysterectomy removes the risk completely.

Can I get pregnant with fibroids?

Many women do. Fibroids that distort the womb cavity can reduce fertility or raise miscarriage risk, and removal may be considered. Discuss your plans before choosing between treatments.

Clinical context

Working up uterine fibroids (symptomatic) properly

A condition like uterine fibroids (symptomatic) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a أمراض النساء specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or أمراض النساء specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
