تمزق الكفة المدورة للكتف

A rotator cuff tear is damage to one or more of the four tendons that stabilise the shoulder. It follows injury or gradual wear, and is most common in adults over forty and in overhead workers and athletes.

عن تمزق الكفة المدورة للكتف

ما تحتاج معرفته

Typical symptoms are pain at the front or side of the shoulder, night pain when lying on it and weakness lifting the arm or reaching overhead. Diagnosis is by examination, ultrasound or MRI, which show whether the tear is partial or full thickness and how large it is. Many tears are small and cause few symptoms, but they can enlarge over time.

Partial tears and many small tears are treated with rest, physiotherapy, anti-inflammatory medicines and sometimes corticosteroid injections. Surgery is for full-thickness tears with weakness, acute tears in active people and when physiotherapy fails. The tendon is reattached to bone, usually arthroscopically, followed by weeks in a sling and months of rehabilitation. Massive irreparable tears may need other options such as tendon transfer.

الفحوصات قبل السفر

فحوصات يجب إجراؤها مسبقاً

  • MRI or ultrasound report of the shoulder stating which tendons are torn, tear size, retraction and muscle fatty degeneration.
  • Plain shoulder X-rays, which can show arthritis or high-riding humeral head.
  • Records of physiotherapy and injections tried, with dates and results.
  • A note of when the tear began, whether it followed an injury, and your dominant arm.
  • Your medical history, especially diabetes, smoking and blood-thinning medicines, which affect tendon healing and surgery.

رأي ثانٍ

متى تطلب رأياً آخر

  • You have had a sudden injury with loss of the ability to lift your arm, where early repair gives the best results.
  • Your tear is described as massive or irreparable and you are offered options ranging from debridement to reverse replacement.
  • You have been advised to have surgery for a small partial tear that has not been tried with physiotherapy.
  • You have had a previous repair that has failed or re-torn.

الحديث مع طبيبك

أسئلة يجب طرحها

  1. How big is my tear, is it partial or full thickness, and has the muscle been damaged beyond repair?
  2. Would physiotherapy be reasonable first, and how long should I try it?
  3. Is arthroscopic repair possible, and what is the chance the tendon heals?
  4. If the tear cannot be repaired, what are the other options, such as tendon transfer or reverse shoulder replacement?
  5. How long will I wear a sling and when can I return to work and driving?
  6. What happens if I choose not to have surgery?

التوقعات

تشخيص واقعي

Many people with small or partial tears improve with physiotherapy and injections. Arthroscopic repair relieves pain in most patients and restores function in many, but tendon healing is less reliable in large tears, older patients, smokers and those with fatty muscle changes. Recovery takes several months, with full strength sometimes needing up to a year.

04 · الأسئلة الشائعة

أسئلة شائعة حول تمزق الكفة المدورة للكتف

Do all rotator cuff tears need surgery?

No. Many partial and small tears settle with physiotherapy and injections. Surgery is more likely if you have an acute traumatic full tear, weakness, or persistent pain despite treatment.

Can a torn rotator cuff heal on its own?

Tendon tears do not usually heal fully without repair, but symptoms can improve as the other muscles compensate. Tears can enlarge over time.

How long is recovery after repair?

A sling is usually worn for several weeks, with physiotherapy over three to six months. Strength can keep improving for up to a year.

Can the tendon tear again?

Yes, re-tear rates are higher with large tears, older age and smoking. Following the rehabilitation plan carefully lowers the risk.

Clinical context

Working up تمزق الكفة المدورة للكتف properly

A condition like تمزق الكفة المدورة للكتف is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
