عن ptosis (drooping eyelid)

ما تحتاج معرفته

The eyelid sits lower than normal and may cover part of the pupil, giving a tired look and causing a tilted-back head posture or brow strain. Causes include ageing, contact lens use, eye surgery, myasthenia gravis, nerve palsy and congenital weakness. Examination measures eyelid height and muscle function, and a visual field test shows whether sight is affected. Sudden ptosis needs urgent medical review.

Treatment depends on the cause. Underlying conditions such as myasthenia are treated first. Mild ptosis may be managed with eyelid-lifting drops or special spectacles. Surgery shortens or tightens the eyelid-lifting muscle, or for very weak muscles suspends the lid from the brow using a sling. In children, surgery is done early if vision is at risk of lazy eye. Some adults travel abroad for oculoplastic surgery.

الفحوصات قبل السفر

فحوصات يجب إجراؤها مسبقاً

  • Frontal photographs of the eyes, including old photographs to show whether the droop is recent.
  • Measurements of eyelid height and levator muscle function, with visual field testing if your sight is affected.
  • Neurological or myasthenia assessment if the droop varies through the day or came on suddenly.
  • Pupil and eye movement findings to exclude nerve palsies, with any MRI or blood tests already done.
  • Eye surface and dry eye assessment, since surgery can worsen dryness.

رأي ثانٍ

متى تطلب رأياً آخر

  • The droop appeared suddenly, varies through the day or is accompanied by double vision, which needs medical rather than surgical review first.
  • A cosmetic eyelid lift is offered but the cause is muscle weakness needing a different operation.
  • You have had surgery before and the lids are uneven.
  • Your child has ptosis and the pupil is partly covered, risking a lazy eye.

الحديث مع طبيبك

أسئلة يجب طرحها

  1. What is causing my ptosis: muscle stretching, nerve or muscle disease, or something present from birth?
  2. What level of muscle function is there, and which operation suits me?
  3. Are there risks to my eye surface, such as dryness or not closing fully?
  4. How likely is it that my lids will be even after surgery, and will I need a second procedure?
  5. Does my sight need to be checked first, or is the child at risk of amblyopia?
  6. How long until swelling and bruising settle?

التوقعات

تشخيص واقعي

Surgery corrects most cases of ptosis with a good functional and cosmetic result, provided the cause is identified first. Some people need a touch-up for uneven lids or under- or over-correction. In children, early correction prevents a lazy eye if the droop blocks the pupil. Results depend on how strong the lifting muscle is.

Does a drooping eyelid always need surgery?

No. Mild ptosis with no effect on vision can be left. Surgery is considered for blocked sight, tiredness or appearance.

Could it be a sign of something serious?

Occasionally. A sudden droop or double vision can signal a nerve or muscle disease, so see a doctor promptly.

Will both eyes match afterwards?

Surgeons aim for symmetry, but small differences are common, and some people need an adjustment.

Can ptosis affect a child's vision?

Yes. If the lid covers the pupil, the brain may stop developing normal sight in that eye, so early treatment is important.

Clinical context

Working up ptosis (drooping eyelid) properly

A condition like ptosis (drooping eyelid) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a طب العيون specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or طب العيون specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
