A lump in the neck is often the first sign, along with a blocked nose, nosebleeds, ear fullness, hearing loss or double vision. Diagnosis needs an endoscopic biopsy, MRI and PET-CT for staging, with Epstein-Barr virus DNA levels in the blood used to monitor response. It is generally sensitive to radiotherapy, and early stages have a good outlook.

Radiotherapy is the main treatment, and intensity-modulated radiotherapy (IMRT) spares nearby structures such as salivary glands and the brainstem. Chemotherapy is given alongside radiotherapy for stages beyond the earliest. Surgery has a limited role, mainly for recurrence. Immunotherapy is used for advanced or recurrent disease. Patients travel abroad for IMRT or proton-beam therapy at specialist cancer centres.

  • Biopsy pathology report, including the type and EBV testing, with slides for review.
  • MRI of the nasopharynx and neck and a PET-CT or CT of the chest and abdomen, which are usually requested for staging.
  • Plasma EBV DNA levels, if measured, at diagnosis.
  • Hearing tests, dental assessment and kidney function and blood counts before treatment.
  • A nasal endoscopy report and details of neck lumps and cranial nerve symptoms.

  • The stage is advanced and different treatment plans have been proposed, including induction chemotherapy.
  • You have been told the cancer is inoperable and want to confirm that radiotherapy is the right treatment.
  • Cancer has returned after radiotherapy and options such as surgery or re-irradiation are being discussed.
  • Distant spread has been found and immunotherapy or trials are being considered.

  1. What is my exact stage, and what did the EBV DNA level show?
  2. What combination of radiotherapy and chemotherapy do you recommend?
  3. What side effects should I expect, such as dry mouth, hearing loss and swallowing problems?
  4. What dental and nutritional care do I need before starting?
  5. How will response be checked after treatment?
  6. What is the plan if the cancer returns?

Nasopharyngeal carcinoma is sensitive to radiotherapy, and early-stage disease is cured in most patients. Outcomes are good for localised disease and fall with larger tumours, neck node involvement and distant spread. Combined chemoradiotherapy has improved results for locally advanced disease. Long-term effects of radiation need management, and regular follow-up is required.

Is surgery used for nasopharyngeal cancer?

Surgery is rarely the main treatment because of the location. Radiotherapy, usually with chemotherapy, is the standard. Surgery may be used for selected recurrences.

Is the cancer linked to Epstein-Barr virus?

Yes, most cases in endemic regions are linked to EBV. Blood EBV DNA can help with diagnosis, tracking response and detecting relapse.

What are the long-term effects of radiotherapy?

Common effects include dry mouth, hearing changes, ear fluid, tooth problems and neck stiffness. Dental care, swallowing exercises and regular follow-up reduce these.

Can it come back?

Yes, mostly in the first few years, at the original site, in the neck, or elsewhere. Close follow-up includes endoscopy, scans and sometimes EBV DNA tests.

Clinical context

Working up nasopharyngeal carcinoma properly

A condition like nasopharyngeal carcinoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a علم الأورام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or علم الأورام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
