There are usually no symptoms. It is identified through semen analysis, with hormone tests, genetic tests and scrotal ultrasound to look for causes such as varicocele, hormonal disorders, blockage or genetic conditions. Results can change between samples, so tests are repeated. Causes range from reversible to permanent.

Treatable causes such as infections, hormone problems or varicocele are addressed first, along with lifestyle changes. Where sperm quality is low, IUI or IVF is used, and ICSI injects a single sperm directly into an egg, with IMSI selecting sperm under higher magnification. If no sperm are in the ejaculate, TESE or micro-TESE retrieves them from the testis. Treatment abroad is often considered for ICSI cycles and surgical sperm retrieval.

  • Two or more semen analyses with count, motility and morphology, and the abstinence interval before each.
  • Hormone tests including FSH, LH and testosterone, and scrotal ultrasound reports.
  • Genetic tests where indicated, such as karyotype and Y-chromosome microdeletion analysis.
  • Records of any testicular biopsy or sperm retrieval, with sperm found and whether it was frozen.
  • The partner's fertility results and any previous IVF or ICSI cycle details, such as fertilisation and embryo quality.

  • No sperm were found on analysis and nobody has explained whether the cause is blockage or poor sperm production.
  • Testicular sperm extraction has failed once and you want to know if micro-TESE is worth trying.
  • Repeated ICSI cycles have failed and the sperm quality tests have not been reviewed.
  • You have not had genetic testing before treatment, despite a very low count.

  1. What is the cause of the problem, and is it treatable?
  2. Which technique, ICSI, IMSI or surgical sperm retrieval, fits my results?
  3. What is the chance of finding sperm, and what are the surgical risks?
  4. Could my genetic result affect a baby?
  5. What is the realistic success rate per cycle given my partner's age?
  6. What lifestyle and medicine changes could improve my sperm?

The chance of a baby depends largely on the female partner's age and ovarian reserve, and on the amount of usable sperm can be obtained. ICSI can achieve fertilisation with very few sperm, and sperm can often be retrieved surgically even in non-obstructive azoospermia, though not always. Success rates per cycle are typically higher in younger women. Some men have genetic causes that could be passed on.

What is ICSI?

ICSI injects a single sperm directly into an egg during IVF. It overcomes very low counts, poor movement or abnormal shape.

What if there are no sperm in my semen?

Sperm may still be found in the testicle through TESE or micro-TESE. Chances depend on the cause, and are highest with blockages.

Can lifestyle changes help?

They may help. Stopping smoking, reducing alcohol, maintaining a healthy weight and avoiding heat and anabolic steroids can support sperm quality, but severe problems still need assisted conception.

Clinical context

Working up male factor infertility properly

A condition like male factor infertility is rarely well-served by jumping straight to treatment shopping. The sequence below is what a الخصوبة وأطفال الأنابيب specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
