Lumbar Spinal Stenosis

Lumbar spinal stenosis is a narrowing of the spinal canal in the lower back that puts pressure on the nerves. It is usually caused by age-related wear and mainly affects people over 60.

The hallmark symptom is leg pain, heaviness or weakness when walking or standing that eases when sitting or leaning forward, called neurogenic claudication. Back pain and numbness may also occur. Diagnosis is by examination and MRI or CT. Symptoms usually progress slowly and many people remain stable for years.

Treatment begins with physiotherapy, exercise, pain relief and epidural steroid injections. Surgery is considered when symptoms limit daily life or nerve weakness appears. Decompression, or laminectomy, removes bone and tissue pressing on the nerves, and often provides good relief of leg symptoms. Spinal fusion is added when the spine is unstable, such as with spondylolisthesis. Treatment abroad is sometimes considered for planned decompression or fusion.

  • MRI of the lumbar spine showing which levels are narrowed and how severe the compression is, and CT if bone detail is needed.
  • Standing X-rays, including flexion and extension views, to look for slippage (spondylolisthesis) or instability.
  • A walking-distance history, noting how far you can walk before symptoms start and how quickly they ease on sitting.
  • Records of physiotherapy, epidural injections and medicines tried, with dates and response.
  • Bone density result if fusion is considered, and heart and diabetes records relevant to anaesthesia.

  • Fusion is recommended and your X-rays show no slippage or instability, which is the usual reason for fusion.
  • Surgery is advised for narrowing on MRI alone, although your symptoms are mild.
  • You have progressive leg weakness, foot drop or bladder changes, which require urgent specialist review.
  • Multiple levels are involved and you want to compare a limited with an extensive operation.

  1. Which levels are narrowed, and do they explain my symptoms?
  2. Is decompression alone enough, or do I need fusion?
  3. What improvement in walking distance can I realistically expect?
  4. What are the risks of nerve injury, infection or the problem recurring at an adjacent level?
  5. How long is the hospital stay and recovery, and when can I fly home?
  6. What exercises and walking aids can help while I decide?

Many people live comfortably with mild stenosis using exercise and activity changes. When symptoms limit walking, decompression surgery improves leg pain and walking distance in most patients, though back pain may be less predictable. Benefits tend to wane for some over five to ten years. Outcomes depend on age, general health and whether a fusion is needed, which lengthens recovery.

Will stenosis get worse?

Often it stays stable or progresses slowly. Many people do well with exercise and activity changes for years.

Is fusion always needed?

No. Decompression alone is enough for most people. Fusion is considered when there is slippage, instability or deformity.

Can I fly or travel with stenosis?

Yes. Long flights can be uncomfortable, so choose an aisle seat, move often, and avoid heavy luggage. After surgery, wait until your surgeon agrees.

Clinical context

Working up lumbar spinal stenosis properly

A condition like lumbar spinal stenosis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
