Liver Metastases (Colorectal)

Colorectal liver metastases are secondary tumours in the liver that have spread from bowel cancer. The liver is the most common site of spread from colon and rectal cancer.

They are often found on follow-up scans, and may be silent or cause abdominal discomfort, weight loss or abnormal liver blood tests. Diagnosis is by contrast CT, MRI, sometimes PET-CT, with biopsy when needed. Number, size and position of the tumours determine whether they can be removed. Without treatment they usually grow and spread.

Treatment is planned by a multidisciplinary team. When tumours can be removed, hepatectomy (liver resection) offers the best chance of long-term control, often combined with chemotherapy. Ablation can treat small lesions. Y-90 radioembolisation (SIRT) delivers radiation directly to liver tumours when surgery is not possible. Chemotherapy and targeted therapy can shrink tumours first. Treatment abroad is often considered for complex liver surgery or SIRT.

  • CT and MRI liver scans showing the number, size and position of metastases, and their relation to blood vessels and the remaining liver volume.
  • PET-CT or chest CT to confirm there is no spread elsewhere.
  • Primary tumour pathology including stage and molecular tests (RAS, BRAF, MSI), plus CEA blood levels over time.
  • Records of chemotherapy given: drugs, number of cycles, dates, and how the liver tumours responded.
  • Liver function tests, and notes on any prior liver or bowel surgery.

  • You have been told the liver metastases are inoperable, but no specialist liver surgery team has reviewed the scans.
  • Chemotherapy has shrunk the tumours and you want to know if surgery is now possible.
  • Surgery and bowel treatment are being planned separately and you want a combined strategy.
  • Cancer has returned in the liver after surgery and you want to hear about repeat resection, ablation or radioembolisation.

  1. Can all the liver metastases be removed or treated, leaving enough healthy liver?
  2. Do I need chemotherapy before or after surgery, and which drugs?
  3. Are ablation or Y-90 radioembolisation better for my tumours than resection?
  4. What do my RAS and BRAF results mean for treatment choices?
  5. What is the chance of recurrence, and how often will I be scanned?
  6. What are the risks of liver surgery and how long is recovery?

Surgery to remove liver metastases offers a real chance of long-term survival, and a significant minority of carefully selected patients are cured. Outcomes depend on the number and size of tumours, whether the bowel cancer is controlled, tumour genetics and response to chemotherapy. Recurrence is common, but further liver treatments are often possible. When the tumours cannot be removed, treatment aims to control disease.

Can liver metastases be cured?

In selected patients whose tumours can all be removed or ablated, long-term survival and sometimes cure is possible. Chemotherapy is often combined with surgery.

Can inoperable tumours become operable?

Sometimes. Chemotherapy can shrink tumours enough for surgery, and techniques such as portal vein embolisation can enlarge the healthy liver.

What proportion of the liver can be removed?

The liver regenerates, but enough healthy liver must remain, generally around 25 to 30 per cent in a normal liver. Scans are used to calculate this.

Clinical context

Working up liver metastases (colorectal) properly

A condition like liver metastases (colorectal) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a علم الأورام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or علم الأورام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
