Kidney Stones (Large / Staghorn)

Large and staghorn kidney stones are big deposits that fill part or all of the kidney's collecting system, often shaped like a stag's antler. They are frequently linked to recurrent infection.

Symptoms include flank pain, blood in the urine, repeated urinary infections and, at times, fever or reduced kidney function. A CT scan shows the size and shape of the stone. These stones rarely pass on their own and, left untreated, can damage the kidney and cause serious infection.

Treatment is active removal. Percutaneous nephrolithotomy (PCNL) is the standard approach for large stones, using a small keyhole in the back to break up and extract the stone. Mini-PCNL uses a narrower tract. Retrograde intrarenal surgery (RIRS) with a flexible scope suits selected moderate-sized stones, sometimes in stages. Infection is treated first, and the aim is to clear all fragments. Treatment abroad is often considered for PCNL.

  • CT scan of the kidneys with stone volume, position and the shape of the collecting system, plus kidney function from a renal scan if one kidney is weak.
  • Urine culture and sensitivity results, because staghorn stones are often linked to infection and antibiotics are needed before surgery.
  • Blood tests: creatinine, eGFR, calcium, uric acid and clotting profile, plus a list of any blood thinners.
  • Stone analysis if you have passed fragments before, and a record of earlier stone operations.
  • Your history of recurrent urinary infections, diabetes, spinal deformity or bowel surgery, which affect surgical planning.

  • You have been advised to leave a staghorn stone alone, which can lead to infection and loss of kidney function.
  • Only shockwave therapy has been offered for a very large stone, which usually needs a combined approach.
  • You have had earlier failed stone surgery and want to know if a stone-free result is realistic.
  • Your kidney function is already reduced and you want to confirm the safest way to clear the stone.

  1. How large is the stone, and is it a full staghorn?
  2. Would PCNL, mini-PCNL or ureteroscopy suit my stone, and how many sessions might I need?
  3. What is the chance of being completely stone-free after the first operation?
  4. What are the risks of bleeding and infection, and how are they managed?
  5. Will I need a stent or a nephrostomy tube, and for how long?
  6. How can I reduce the chance of the stones returning?

Most people achieve a stone-free kidney after one or two procedures, but staghorn stones are more complex and fragments may be left behind. Success depends on stone volume, kidney anatomy and infection. Recurrence is common, especially with infection stones, so follow-up scans and prevention measures matter. Untreated staghorn stones can cause serious infection and progressive loss of kidney function.

Why not just use shockwave therapy?

Large and staghorn stones are too bulky for shockwaves to clear reliably, and fragments can block the ureter. PCNL removes them directly through a small tract in the back.

Will I need more than one operation?

Sometimes. Staghorn stones can need a second session for residual fragments. Your surgeon will plan this using the scan.

Can I leave a staghorn stone untreated?

It is not advised. Untreated staghorn stones commonly cause recurrent infections and gradual loss of kidney function, and can occasionally cause serious sepsis.

Clinical context

Working up kidney stones (large / staghorn) properly

A condition like kidney stones (large / staghorn) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a المسالك البولية specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or المسالك البولية specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
