Knee Cartilage Defect

A knee cartilage defect is a localised area of damage to the smooth cartilage that covers the bone ends in the knee joint. It can follow injury or wear and often affects active adults.

Symptoms include knee pain with activity, swelling, catching or locking, and a feeling of giving way. Diagnosis is by examination and MRI, which shows the size and depth of the defect, and sometimes by arthroscopy. Cartilage has little ability to heal, so untreated larger defects can contribute to early osteoarthritis.

Small or low-symptom defects are treated with physiotherapy, activity changes and injections. For persistent symptoms, microfracture stimulates a repair of fibrocartilage by drilling into the bone and suits smaller defects. Larger defects may be treated with cell-based restoration such as MACI, a two-stage procedure using the patient's own cartilage cells, or with grafts. Treatment abroad is sometimes considered for MACI or other restorative techniques.

  • MRI of the knee showing the defect's size, depth, location and the state of the surrounding cartilage, meniscus and ligaments.
  • Standing weight-bearing X-rays, plus long-leg alignment views if you have bowed legs or knock knees.
  • Operation notes from any earlier knee arthroscopy, microfracture or cartilage procedure.
  • Records of physiotherapy, injections and pain and activity scores, plus your occupation and sporting demands.
  • Your height, weight and BMI, and smoking status, which influence the likelihood of cartilage healing.

  • You are told the only option is a knee replacement but you are young and the damage is a single defect.
  • Microfracture failed and you are being offered it again, rather than a restorative option.
  • A procedure is advised without checking leg alignment, ligament stability or meniscal damage that could cause it to fail.
  • The defect is large and you want to compare MACI with osteochondral grafting.

  1. How large and deep is my defect, and are the nearby surfaces healthy?
  2. Would microfracture, grafting or cell-based repair suit my defect size and age?
  3. Do my leg alignment or ligaments need correcting at the same time?
  4. How long will I be on crutches, and when can I return to sport or work?
  5. What is the realistic chance of pain relief, and how long does the repair last?
  6. What happens if the repair fails?

Outcomes depend on defect size, location, patient age, body weight and whether alignment and ligaments are also addressed. Microfracture can give good short-term relief in small defects but tends to wear after several years. Cell-based and graft procedures generally last longer in larger defects. Recovery is slow, often six to twelve months before sport, and cartilage repair does not always prevent later arthritis.

Can cartilage heal itself?

Adult cartilage has little ability to heal because it has no blood supply. Small defects may become symptom-free with rehabilitation, but larger ones often need a restoration procedure.

Will I get arthritis later?

A defect left untreated can increase the risk of arthritis. Repair aims to protect the joint, though it does not guarantee that arthritis never develops.

How long is recovery after MACI?

MACI is staged: cells are harvested first, then implanted weeks later. Weight-bearing is limited for weeks and return to sport commonly takes 9 to 12 months or more.

Clinical context

Working up knee cartilage defect properly

A condition like knee cartilage defect is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
