Pyloric Stenosis (Infant)
Pyloric stenosis is a condition in which the muscle at the outlet of a baby's stomach thickens and blocks milk from passing into the intestine. It usually appears in the first weeks of life, more often in boys.

A baby typically feeds well at first, then begins forceful projectile vomiting after feeds, remains hungry, and loses or fails to gain weight. Dehydration can follow. Doctors may feel an olive-shaped lump in the abdomen, and an ultrasound confirms the thickened pylorus. Blood tests check for salt and acid balance. Without treatment the baby becomes progressively weaker.
Treatment is surgical, but first the baby is stabilised with intravenous fluids and correction of blood salt imbalances. A pyloromyotomy, in which the thickened muscle is split, is then performed, usually through keyhole or a small cut near the navel. Feeding restarts within hours and most babies recover quickly. Because it is time-sensitive, treatment is normally given at the nearest capable paediatric surgical centre rather than abroad.
- Ultrasound report measuring pyloric muscle thickness and channel length, which usually confirms the diagnosis.
- Blood test results for sodium, potassium, chloride and bicarbonate, as vomiting causes a typical low-chloride alkalosis that must be corrected first.
- Your baby's birth weight, current weight and a feeding and vomiting log with times and volumes.
- Any medicines your baby has received, including antibiotics such as erythromycin, and notes on prematurity or other conditions.
- Recent hospital discharge or emergency notes, and vaccination records.
- Your baby is vomiting forcefully after every feed and has not yet had an ultrasound or blood tests.
- A diagnosis has been made but nobody has checked hydration and blood salts before surgery was proposed.
- Your baby has other conditions, such as prematurity or heart disease, which may change anaesthetic planning.
- Vomiting continues beyond a few days after surgery and you are told this is normal without any review.
- Does the ultrasound confirm pyloric stenosis, and could anything else cause these symptoms?
- Are my baby's blood salts and hydration safe for surgery, and how long will correction take?
- Will the operation be keyhole or open, and how often has the team performed it in infants?
- When can feeding restart after surgery, and what vomiting is expected?
- Are there signs of wound infection or a complication I should watch for at home?
- When is it safe to travel by air after the operation?
Outcomes are excellent. Once dehydration is corrected and the muscle is divided, babies feed normally within a day or two, and long-term problems are rare. Some occasional vomiting in the first days is common. Serious complications such as incomplete division or wound infection are uncommon. The condition does not recur, and growth normally catches up quickly.

Is pyloric stenosis my fault or caused by feeding?
No. It is not caused by anything parents do. It is more common in first-born boys and runs in some families.
Does my baby always need surgery?
In nearly all cases yes. Pyloromyotomy divides the thickened muscle and is curative. Some babies are treated with medicine in rare settings, but this takes much longer.
Will my baby have problems later?
Long-term problems are rare. Most babies feed normally within days and grow well. A little vomiting in the first day or two after surgery is common.
Clinical context
Working up pyloric stenosis (infant) properly
A condition like pyloric stenosis (infant) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة الأطفال specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or جراحة الأطفال specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
