IgA Nephropathy (Progressed)

IgA nephropathy is a kidney disease in which immunoglobulin A deposits build up in the filtering units of the kidney, causing inflammation. Progressed disease means kidney function has fallen significantly.

Early disease may cause only blood in the urine, often after a throat infection, or protein in the urine. As it progresses, people may develop high blood pressure, swelling, tiredness and falling kidney function. Diagnosis is by urine and blood tests and, usually, a kidney biopsy. Course varies widely: some people remain stable for decades, while others advance to kidney failure over years.

Care aims to slow decline with strict blood pressure control, ACE inhibitors or ARBs, reduced salt and, in selected cases, corticosteroids or newer immune-directed medicines. Once kidney failure develops, dialysis or a kidney transplant is needed. A deceased donor transplant is an option for suitable patients, although IgA deposits can recur in the new kidney. Treatment abroad is sometimes considered when transplant access is limited locally.

  • Kidney biopsy report, including the Oxford MEST-C scores, the number of glomeruli sampled and any crescents or scarring.
  • Serial creatinine and eGFR results over at least a year, to show how fast kidney function is declining.
  • Urine protein-to-creatinine ratio or 24-hour protein results, and blood pressure readings over time.
  • Treatment history, including ACE inhibitors, ARBs, steroids or other immunosuppression, and how you responded.
  • Transplant work-up so far: blood group, tissue typing, infection screening, and any donor offer or living-donor evaluation.

  • You have been told dialysis or transplant is inevitable but no recent biopsy or full review of immune treatment options has taken place.
  • Your protein loss remains high despite maximal blood pressure and kidney-protective medicines.
  • A relative is willing to donate and you have not had compatibility and risk assessed by a transplant team.
  • IgA nephropathy has been reported in your transplanted kidney, or you are unsure how likely recurrence is.

  1. What stage is my kidney disease, and how quickly is my eGFR falling?
  2. Is any active inflammation still present that treatment could slow, or is the damage mostly scarring?
  3. When should I start preparing for dialysis access or a pre-emptive transplant?
  4. Could a living donor be an option, and what is the chance that IgA nephropathy returns in the new kidney?
  5. What anti-rejection medicines would I need, and how will they be followed up when I return home?
  6. What can I do now about blood pressure, salt and protein intake to protect remaining function?

IgA nephropathy varies widely. Some people keep stable function for decades, while others reach kidney failure over ten to twenty years. Heavy proteinuria, high blood pressure and scarring on biopsy predict faster decline. Once failure is reached, a transplant usually gives better survival and quality of life than dialysis. The disease can recur in the graft, though it rarely causes early loss.

Will IgA nephropathy always lead to kidney failure?

No. Many people keep stable function for decades. Heavy protein loss, high blood pressure and scarring on biopsy make progression more likely.

Can IgA nephropathy come back after a transplant?

Yes, it can recur in the new kidney, often years later. It causes graft loss in only a minority of recipients, and it is not usually a reason to avoid transplant.

Is a transplant better than dialysis?

For most suitable patients, a transplant gives longer survival and a better quality of life than dialysis. Suitability depends on age, heart health and other illnesses, which a transplant team will assess.

Clinical context

Working up iga nephropathy (progressed) properly

A condition like iga nephropathy (progressed) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a زراعة الأعضاء specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or زراعة الأعضاء specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
