Hemifacial Spasm

Hemifacial spasm is a condition of involuntary, repeated twitching of the muscles on one side of the face. It is most often caused by a blood vessel pressing on the facial nerve where it leaves the brainstem.

The twitching usually begins around the eyelid and spreads to the cheek and mouth, and may be triggered by stress or fatigue. Persisting over time, it can interfere with vision and social life. Diagnosis is clinical, with MRI of the brain to show the offending vessel and exclude tumours or other causes.

Botulinum toxin injections into the affected muscles every few months are the usual first treatment and are effective for most people. Some medicines have a limited effect. Microvascular decompression is a neurosurgical operation that moves the vessel away from the nerve and can cure the spasm, so it is considered when injections stop working or are not tolerated. It requires an experienced neurosurgeon, and patients may seek this care abroad.

  • MRI brain with high-resolution sequences (such as CISS or FIESTA) showing the facial nerve and any vessel compressing it.
  • A neurology letter confirming the spasm is hemifacial and not facial tics, blepharospasm or a seizure disorder.
  • Records of botulinum toxin treatment: dates, dose, sites injected, how long it worked and any side effects.
  • Hearing test results, since the nerve lies close to the hearing nerve and surgery can affect hearing.
  • Any history of facial weakness, a previous Bell's palsy, or medicines used such as carbamazepine.

  • The MRI does not clearly show a compressing vessel, or a tumour or other lesion has not been excluded.
  • Botulinum toxin still helps and surgery is recommended without discussing its risk to hearing and facial nerve.
  • Toxin has become less effective, or its side effects are a problem, and you are considering decompression.
  • The spasm affects both sides or involves other nerves.

  1. Is the cause a blood vessel pressing on the nerve, and is it visible on MRI?
  2. Should I continue botulinum toxin, and how often?
  3. What is the success rate of microvascular decompression in your experience?
  4. What are the risks to hearing and facial movement?
  5. How long until the spasm stops after surgery?
  6. What happens if the spasm returns?

Hemifacial spasm is not dangerous but tends to persist and worsen without treatment. Botulinum toxin controls it in most people, with repeat injections every few months. Microvascular decompression offers a chance of long-term cure in a large majority of selected patients, though relief can take days to weeks and sometimes months. Risks include hearing loss, facial weakness and cerebrospinal fluid leak, and recurrence is possible. Results depend on surgeon experience and finding a clear compressing vessel.

Will hemifacial spasm go away by itself?

It rarely resolves on its own and may slowly worsen. Treatment controls it effectively in most people.

How do botulinum toxin injections work?

They weaken the overactive muscles for around three months, then repeat injections are needed. Side effects can include temporary facial weakness or drooping.

Is surgery a cure?

Microvascular decompression can cure the spasm in most well-selected patients by moving the vessel off the nerve, but carries small risks to hearing and facial movement.

Clinical context

Working up hemifacial spasm properly

A condition like hemifacial spasm is rarely well-served by jumping straight to treatment shopping. The sequence below is what a الأعصاب وجراحة الأعصاب specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
