Femur Fracture

A femur fracture is a break in the thigh bone, the largest bone in the body. It usually follows a high-energy injury in younger people or a fall in older adults with fragile bones.

The leg is very painful, often shortened or turned outwards, and cannot bear weight. Blood loss can be significant. Diagnosis is by X-ray, sometimes with CT to plan fixation, and the nerves and blood supply to the leg are checked. Healing generally takes several months, and recovery depends on the fracture type, other injuries and the person's overall health.

Emergency care involves pain relief, fluids and stabilising the limb. Most femoral shaft fractures are fixed with an intramedullary nail, while others need plates and screws, an approach known as open reduction and internal fixation (ORIF). Physiotherapy begins early to restore walking. Complex, infected or poorly healed fractures sometimes need revision surgery, which is the usual reason for seeking specialist orthopaedic care elsewhere.

  • The original X-rays or CT scans, ideally as image files, showing the exact site and pattern of the break.
  • The hospital discharge summary and operation notes, including any plates, nails or external frame already fitted.
  • Blood results, including haemoglobin, and a record of any transfusion, plus clotting-prevention treatment given.
  • Notes on any other injuries, wound or skin condition over the thigh, and signs of infection if surgery has already been done.
  • A list of regular medicines and other conditions such as diabetes or osteoporosis that affect bone healing.

  • The fracture is open, the wound is not healing, or there are signs of infection after a previous fixation.
  • The bone has not shown healing on X-ray several months after surgery, which suggests a non-union.
  • You are told the break is complex, near the hip or knee joint, or has multiple fragments, and surgical options differ between surgeons.
  • The limb is shorter, rotated or painful after fixation and you are told this is normal.

  1. Where exactly is the fracture, and what type is it?
  2. Which fixation do you recommend, a nail, plate or other, and why for this break pattern?
  3. When can I start putting weight on the leg?
  4. What are the risks of infection, delayed healing or leg length difference?
  5. Will the metalwork need removing later, and how will healing be checked on X-ray?
  6. What physiotherapy will I need, and for how long?

Most femoral shaft fractures treated with a nail or plate heal in about three to six months, and many people regain good walking function. Outcome depends on the fracture pattern, whether the skin was broken, how soon it was fixed, age, smoking, diabetes and bone quality. Complications such as infection, delayed healing or non-union occur in a minority and may need further surgery. Rehabilitation takes several months.

Is surgery always needed?

Most adult femoral shaft or hip-region fractures need surgery to fix the bone, because a thigh bone seldom heals well in a cast and long bed rest carries serious risks. Your surgeon decides based on the fracture type.

When can I walk on it?

It depends on the fracture and the fixation. With a stable nail, many people put some weight on the leg within days to weeks, but your surgeon will give specific limits.

Can I fly after a femur fracture?

Air travel is often possible a few weeks after fixation if you are stable, but clot risk is raised, so you will usually need your surgeon's agreement and clot prevention advice.

Will the metalwork need removing?

Usually not. Nails and plates are commonly left in place unless they cause pain or infection.

Clinical context

Working up femur fracture properly

A condition like femur fracture is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
