Early disease often causes no symptoms, which is why regular retinal screening matters. Later signs include blurred vision, floaters, dark patches or sudden loss of vision from bleeding or retinal detachment. Diagnosis uses dilated retinal examination, photography and optical coherence tomography (OCT) to detect macular swelling. Tight control of blood sugar, blood pressure and cholesterol slows progression.

Mild disease is monitored with good diabetes control. Laser treatment seals leaking or abnormal vessels, and anti-VEGF injections into the eye reduce macular swelling and new vessel growth, usually repeated over months. Vitrectomy surgery removes blood or scar tissue from the vitreous when bleeding or retinal detachment occurs. Patients with advanced disease sometimes seek treatment abroad, but follow-up is ongoing.

  • Retinal photographs or a dilated eye examination report stating the stage: mild, moderate, severe non-proliferative or proliferative.
  • Optical coherence tomography (OCT) scans, which show macular swelling and are usually requested to guide injections.
  • Fluorescein angiography images if they have been done.
  • Your HbA1c readings, blood pressure and kidney test results, since all influence progression.
  • A history of previous laser, anti-VEGF injections, eye surgery, and any cataract operation, with dates.

  • Your vision has dropped and you have been told to wait, or injections have not reduced macular swelling.
  • You have been advised vitrectomy for vitreous bleeding or retinal detachment and want to confirm timing and necessity.
  • Retinopathy was found at a late stage because no screening was done earlier.
  • You are offered repeated injections and want the long-term plan and expected number of treatments explained.

  1. What stage is my retinopathy in each eye, and is there swelling in the macula?
  2. Do I need laser, injections, surgery, or monitoring only?
  3. How many injections might I need, and how often are check-ups required?
  4. What vision can I realistically regain, and what loss is permanent?
  5. What can I do about blood sugar and blood pressure to slow the disease?
  6. What symptoms mean I should be seen urgently?

Diabetic retinopathy can often be controlled, and vision loss can usually be prevented or limited when it is caught early and treated promptly. Anti-VEGF injections and laser improve or stabilise sight in many people, while vitrectomy is used for bleeding or detachment. Vision that has already been lost may not return. Long-term outcome depends strongly on blood sugar, blood pressure and cholesterol control, and regular eye screening.

Will I go blind from diabetic retinopathy?

Most serious vision loss can be prevented with regular screening, timely laser or injections, and good control of blood sugar and blood pressure. Vision already lost may not recover, so early treatment matters.

Do anti-VEGF injections cure it?

No, they control swelling and abnormal vessels but do not cure diabetes or the underlying disease. Several injections are often needed over months or years.

When is vitrectomy needed?

It is used when blood does not clear from inside the eye or the retina is pulled away by scar tissue. It removes the blood and scar tissue and allows repair of the retina.

Clinical context

Working up diabetic retinopathy properly

A condition like diabetic retinopathy is rarely well-served by jumping straight to treatment shopping. The sequence below is what a طب العيون specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or طب العيون specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
