It is often seen on pregnancy scans or at birth. The foot is stiff and cannot be moved into a normal position by hand, but the condition is not painful for a baby. Without treatment, a child walks on the side or top of the foot, so correction is started in the first weeks of life.

The Ponseti method is the first-line treatment: gentle weekly manipulation and plaster casts, usually followed by a small tendon release, then a boots-and-bar brace worn for several years to prevent relapse. Most feet are corrected this way. Surgery is reserved for stubborn, relapsed or neglected feet. Treatment abroad is sometimes considered for a specialist paediatric orthopaedic team.

  • Photographs of the feet and a surgeon's description of severity, with the child's age and any previous casts, with dates.
  • A paediatric examination to check for other conditions such as spina bifida, hip problems or genetic syndromes.
  • Foot X-rays or ultrasound if the feet are older, relapsed or atypical.
  • Details of any Achilles tenotomy done and bracing (boots-and-bar) use, with how well it was followed.
  • Walking and gait observations, ideally a short video, for older children.

  • Casting has not corrected the foot after several weeks, or you were told surgery is the only option from the start.
  • The foot is relapsing in the brace, or the child is already walking and has a returning deformity.
  • Your child has other conditions and the foot looks stiff or unusual (an atypical or syndromic clubfoot).

  1. Is this a typical clubfoot, and is Ponseti casting suitable at this age?
  2. How many casts are likely, and will a small Achilles tendon release be needed?
  3. How long must the brace be worn each day, and for how many years?
  4. What signs show the foot is relapsing, and what then?
  5. Will my child be able to walk, run and wear normal shoes?

Ponseti casting corrects the large majority of clubfeet in infants, often more than 90 per cent, when started early and followed by bracing. The main cause of relapse is stopping the brace too soon. Surgery is needed for a minority, and feet associated with other conditions are harder to treat. Most treated children walk, run and play normally, although the treated foot is often slightly smaller.

Does clubfoot need surgery?

Often not. Most feet are corrected with serial plaster casts and, usually, a small heel-cord release. Surgery is reserved for resistant, relapsed or atypical feet.

Can clubfoot come back?

Yes, it can relapse, most often when the brace is not worn as advised. Early signs are caught by regular review, and relapses are usually treated with casts again.

When should treatment begin?

Ideally in the first weeks of life, but casting can still help older infants and children, though it becomes less predictable with age.

Will my child walk normally?

Most treated children walk, run and play sports, although the affected foot and calf are often slightly smaller and may be stiffer.

Clinical context

Working up clubfoot (talipes) properly

A condition like clubfoot (talipes) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
