عن class iii obesity (bmi 40+)

ما تحتاج معرفته

People may have breathlessness, joint pain, reflux, sleep apnoea, fatty liver and type 2 diabetes. Assessment looks at BMI, associated conditions, previous attempts at weight loss, mental health and readiness for lifelong follow-up. Sustained weight loss through lifestyle change alone is unusual at this level.

Diet, activity, behavioural support and weight-loss medicines come first, but metabolic surgery is the most effective long-term treatment for many people. Options include sleeve gastrectomy, Roux-en-Y bypass, one-anastomosis (mini) gastric bypass and SADI-S, some done with robotic assistance. The choice depends on weight, diabetes, reflux and surgeon experience. Lifelong vitamin supplements and check-ups are needed. Treatment abroad is considered for access to bariatric surgery.

الفحوصات قبل السفر

فحوصات يجب إجراؤها مسبقاً

  • Height, weight, BMI and documented attempts at supervised weight loss, with dates and outcomes.
  • Blood tests including HbA1c, lipids, liver enzymes, vitamin D, B12, iron, and thyroid function.
  • Upper endoscopy for reflux and hiatus hernia, and liver ultrasound, as these influence the choice of operation.
  • Sleep study, ECG, echocardiogram and lung function if sleep apnoea, heart or breathing problems are suspected.
  • A list of medicines, especially for diabetes and blood pressure, and any psychological or dietitian assessment.

التوقعات

تشخيص واقعي

Bariatric surgery produces larger and longer-lasting weight loss than diets or medicines, often 25 to 35 per cent of body weight depending on the operation. It commonly improves or resolves type 2 diabetes, high blood pressure and sleep apnoea. Lifelong vitamin supplements and follow-up are needed. Results depend on the operation chosen, eating habits and continued support, and some weight regain is common.

04 · الأسئلة الشائعة

أسئلة شائعة حول class iii obesity (bmi 40+)

Which operation is best for me?

It depends on your reflux, diabetes, weight, eating habits and previous surgery. The sleeve, bypass, mini bypass and SADI-S have different strengths and risks, so the choice is individual.

Will my diabetes go away?

Many people see blood sugar improve markedly, and some achieve remission, especially if diabetes is recent. The effect varies by operation and by how long you have had diabetes.

Do I need vitamins afterwards?

Yes. Lifelong multivitamin and mineral supplements, including vitamin B12, iron, calcium and vitamin D, are usually needed, along with regular blood tests.

When can I fly after surgery?

Most surgeons advise waiting about one to two weeks, to allow for early complications and reduce the risk of clots. Walking, fluids and any prescribed blood thinners help during the flight.

Clinical context

Working up class iii obesity (bmi 40+) properly

A condition like class iii obesity (bmi 40+) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة السمنة specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة السمنة specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
