Choledochal Cyst
A choledochal cyst is an abnormal widening of part of the bile duct, usually present from birth. It is most often found in infancy or childhood, though some people are diagnosed as adults.

عن choledochal cyst
ما تحتاج معرفته
Symptoms include abdominal pain, jaundice, vomiting and sometimes a lump in the upper abdomen; in some people repeated pancreatitis or bile infection is the first sign. Ultrasound often raises suspicion, and MRCP or CT confirms the shape and extent of the cyst. Left in place, the cyst carries a lifelong risk of stones, infection and, in time, bile duct cancer.
Because of the cancer risk, the standard treatment is surgery to remove the cyst, followed by reconstruction of the bile flow to the bowel, called a hepaticojejunostomy. It is usually done by open or keyhole surgery in a centre experienced in bile duct operations. Drainage or antibiotics only treat complications. Treatment abroad is considered when a hepatobiliary or paediatric surgical team with experience in this rare condition is needed.
الفحوصات قبل السفر
فحوصات يجب إجراؤها مسبقاً
- Ultrasound and MRCP (MRI of the bile ducts) showing the cyst's type, size and whether the pancreatic duct joins abnormally.
- Liver function tests, amylase and lipase results, particularly if pain or jaundice episodes have occurred.
- Reports of any past cholangitis, pancreatitis or ERCP, and the dates of those episodes.
- CT of the abdomen if there is concern about thickened cyst walls or a mass, and tumour markers if advised.
- For a child, growth charts, vaccination record and anaesthetic history, along with a recent blood count and clotting tests.
رأي ثانٍ
متى تطلب رأياً آخر
- You were advised to just observe the cyst, since it is not causing symptoms.
- The procedure planned is drainage alone rather than removing the cyst, which is not usually enough on its own.
- Imaging shows a thickened wall, a mass or liver changes, and no hepatobiliary surgeon has reviewed it.

أسئلة يجب طرحها
- Which type of choledochal cyst is this, and is the pancreatic duct involved?
- Will the whole cyst be removed and the bile duct reconnected to the bowel, and by which technique?
- What is the risk of cancer if the cyst is left in place?
- What symptoms after surgery should prompt urgent review, such as fever or jaundice?
- How often will liver tests and scans be needed in the following years?
التوقعات
تشخيص واقعي
Complete removal of the cyst, with rebuilding of the bile drainage, gives very good results in most patients and removes the main cancer risk. Long-term follow-up is still advised, because narrowing at the join, stones or liver problems can occur years later. Outcomes are best when surgery is done before repeated infection or cancer develops.

04 · الأسئلة الشائعة
أسئلة شائعة حول choledochal cyst
Is surgery always needed?
In nearly all cases, yes, even without symptoms. Complete removal is advised because the cyst can cause infection and pancreatitis and carries a raised risk of bile duct cancer over time.
Can the cyst come back?
If the whole cyst is removed, it does not regrow. Rarely, narrowing at the join between duct and bowel or stones can develop years later, so follow-up is advised.
How long is recovery?
A hospital stay is generally around a week, with return to normal activity in a few weeks. Keyhole or robotic approaches may shorten recovery in suitable patients.
Clinical context
Working up choledochal cyst properly
A condition like choledochal cyst is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة الجهاز الهضمي specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or جراحة الجهاز الهضمي specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
