Cervical Myelopathy

Cervical myelopathy is compression of the spinal cord in the neck, most often from age-related wear of the discs and joints. It mainly affects people over 50 and can steadily impair walking and hand function.

Typical symptoms are clumsy or numb hands, difficulty with buttons or handwriting, an unsteady, stiff-legged walk and sometimes urgency or loss of bladder control. Doctors find brisk reflexes and other cord signs on examination, and an MRI scan shows the narrowing of the spinal canal. Without treatment, mild cases may stay stable for a time, but many worsen gradually or in steps.

Mild, stable myelopathy may be watched with regular review and activity advice, although physiotherapy and painkillers do not relieve cord pressure. Surgery to decompress the cord is advised when symptoms are moderate, severe or progressing. The approach may be from the front, as in anterior cervical discectomy and fusion, or from the back. Treatment abroad is usually considered when surgery is wanted soon and a spine centre with experience in cord decompression is needed.

  • MRI of the cervical spine, ideally on a disc or file you can carry, showing cord compression and any signal change within the cord.
  • Standing X-rays of the neck, including flexion and extension views, to check alignment and instability.
  • A neurologist or spine surgeon's note describing hand function, gait, reflexes and any bladder or bowel symptoms, with dates.
  • A CT of the cervical spine is often requested if ligament calcification or bony canal narrowing is suspected.
  • Nerve conduction or evoked-potential studies if the diagnosis is uncertain, plus a list of current medicines and blood thinners.

  • Your MRI shows cord compression but your symptoms are mild, and you are unsure whether to operate now or monitor.
  • Your hands or walking are clearly getting worse over weeks or months, and surgery has not yet been discussed.
  • You have been offered a front-of-neck operation but compression spans several levels, where a back-of-neck approach may be considered.

  1. How many levels of my spine are compressed, and is there signal change in the cord on MRI?
  2. Would a front or back approach suit my spine alignment and number of affected levels, and why?
  3. What happens to my hands and walking if I delay surgery for six months?
  4. What share of my existing weakness or numbness is likely to recover, and what may remain?
  5. What follow-up scans and physiotherapy will I need, and when can I travel home?

Surgery aims mainly to stop further loss of function rather than to restore everything. Many people improve in walking and hand strength, but damage that has been present for long or shows marked cord changes on MRI tends to recover less fully. Earlier decompression, milder symptoms and younger age usually predict better results. Untreated moderate or severe myelopathy often worsens in steps over time.

Is surgery always needed?

Not always. Mild, stable symptoms are sometimes watched with regular review, but moderate or progressive myelopathy is usually treated with decompression because lost function rarely returns fully. Your surgeon bases the advice on symptoms, examination and MRI findings.

Can the problem come back after surgery?

The original compression is relieved, but neighbouring levels can wear further over the years and occasionally cause new symptoms. Keeping follow-up appointments and reporting new hand clumsiness or walking changes early helps.

Can I fly with cervical myelopathy?

Flying itself does not harm the cord, but if you are unsteady you may need assistance, and a collar or neck care advice may apply. After surgery, your surgeon will say when it is safe, often within a few weeks.

Clinical context

Working up cervical myelopathy properly

A condition like cervical myelopathy is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
