Idiopathic Scoliosis (Adolescent)

Adolescent idiopathic scoliosis is a sideways curve of the spine that appears during growth, usually between ten and fifteen years, without a known cause. It is more common and more likely to progress in girls.

Most curves cause no pain and are noticed as uneven shoulders, a prominent shoulder blade or a rib hump when bending forward. Diagnosis is by examination and a standing spine X-ray, with the curve measured as a Cobb angle. The risk of progression depends on the curve size and the amount of growth remaining.

Small curves are monitored with regular check-ups. Curves of roughly 25 to 40 degrees in a growing child are usually treated with a brace worn to limit progression. Larger curves, or ones that progress despite bracing, may need spinal fusion surgery with rods and screws. Families sometimes travel abroad for surgery at specialist paediatric spine centres.

  • Standing full-spine X-rays (PA and lateral) showing the Cobb angle of each curve, with earlier films for comparison.
  • Skeletal maturity assessment, such as Risser sign or hand X-ray, and menstrual history in girls.
  • Records of any bracing, including hours per day worn and compliance.
  • MRI of the spine if there is pain, neurological signs, left-sided curve or unusually early onset.
  • Lung function results if the curve is large, as severe thoracic curves can restrict breathing.

  • Surgery is advised for a curve that has not clearly progressed or while growth remains and bracing has not been tried.
  • The curve is between about 25 and 45 degrees and you want advice on bracing versus surgery.
  • The curve is painful or atypical, suggesting another cause that needs imaging.
  • You are comparing fusion with newer growth-modulating techniques, which suit selected patients.

  1. What is my Cobb angle and what growth is left?
  2. How likely is the curve to progress without treatment?
  3. Is bracing realistic for me, and how effective is it?
  4. If I need surgery, which levels would be fused and what movement is lost?
  5. What are the neurological risks, and how are they monitored during the operation?
  6. When can I return to school, sport and flying?

Many adolescent curves remain small and need only observation. Bracing reduces the chance of progression in growing teenagers with moderate curves when worn as prescribed. Curves that progress beyond roughly 45 to 50 degrees often need fusion surgery, which usually corrects a large part of the curve and allows most teenagers to return to normal activity. Curve size, growth remaining and curve pattern determine the outlook.

Will my curve keep getting worse?

It may progress while growth continues, particularly with larger curves in younger children. Once growth is complete, progression usually slows, though larger curves can still worsen slowly.

Does a brace correct the curve?

A brace aims to stop progression rather than straighten the spine. It works best when worn as prescribed during the growing years.

Will I be able to do sport after surgery?

Most teenagers return to normal activity, including many sports, after recovery. Your surgeon will advise on timing and any contact-sport limits.

Clinical context

Working up idiopathic scoliosis (adolescent) properly

A condition like idiopathic scoliosis (adolescent) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a جراحة العظام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or جراحة العظام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
