Acute Myeloid Leukemia (AML)

Acute myeloid leukaemia is a fast-moving cancer of the bone marrow in which abnormal immature cells crowd out normal blood cells. It mainly affects adults, and the risk rises with age.

Tiredness, breathlessness, infections, bleeding and bruising are common, as healthy blood cells fall. Diagnosis needs blood tests and a bone marrow biopsy, with cytogenetic and molecular tests that classify the subtype and predict response. These results decide treatment intensity, and outlook varies widely with genetic risk and age.

Fit patients receive intensive induction chemotherapy to reach remission, then consolidation chemotherapy or an allogeneic stem cell transplant, usually for intermediate- or high-risk disease. Targeted drugs are added for specific mutations, and older or less fit patients may have gentler combinations. Treatment abroad is usually considered for transplant, with a matched donor and a long period of follow-up in mind.

  • Bone marrow biopsy and flow cytometry reports confirming AML and its subtype.
  • Cytogenetics and molecular panel results such as FLT3, NPM1, IDH1/2 and TP53, which define risk and targeted drug options.
  • Full blood counts and the date of diagnosis, with any infections or transfusions since.
  • Chemotherapy record showing the regimen, dates and remission status after induction.
  • HLA typing of you and any siblings, and donor search results if transplant is planned.

  • Genetic testing was not completed and treatment was chosen without knowing the FLT3, NPM1 or TP53 status.
  • You are in remission and uncertain whether to proceed to allogeneic transplant now or watch.
  • The leukaemia is refractory to treatment or has relapsed.
  • You are older or have other illnesses and have been offered only palliative treatment.

  1. What is my genetic risk group, and what does it mean for treatment?
  2. Should I have a stem cell transplant in first remission, and how was that decided?
  3. Are there targeted drugs for my mutations?
  4. What are the risks of infection, graft-versus-host disease and relapse after transplant?
  5. How will my remission be checked, and how often?
  6. What happens to me if I cannot have intensive treatment?

AML can be cured in some people, particularly younger patients with favourable genetics, but relapse is a real risk. Many adults reach remission with chemotherapy, and an allogeneic transplant improves the chance of lasting remission in intermediate and high-risk disease. Age, other illnesses, mutation profile and response to induction chemotherapy are the main factors determining the outcome.

Does everyone with AML need a transplant?

No. Transplant is generally recommended for intermediate- and high-risk disease in fit patients, while favourable-risk disease may be treated with chemotherapy alone.

Can AML be cured?

Yes, in a proportion of patients, particularly younger people with favourable genetics. Relapse is the main risk and is monitored with regular blood and marrow tests.

How long will I be in hospital?

Intensive chemotherapy usually needs several weeks in hospital for each major cycle because of infection risk. Transplant involves a longer stay and months of close follow-up.

Clinical context

Working up acute myeloid leukemia (aml) properly

A condition like acute myeloid leukemia (aml) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a علم الأورام specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or علم الأورام specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
