The typical sign is a mouth ulcer or sore patch on the tongue that does not heal within three weeks, sometimes with a lump, pain, bleeding or a sore throat or ear. Diagnosis is by biopsy, and MRI or CT and PET scans show size and neck node spread. Early detection greatly improves the chance of cure.

Surgery to remove the tumour (glossectomy) is the usual treatment for oral tongue cancers, with neck dissection to remove lymph nodes, and reconstruction with tissue from another part of the body to preserve speech and swallowing. Radiotherapy, with or without chemotherapy, is added after surgery for higher-risk disease and is the main treatment for base-of-tongue cancers. Specialist head and neck centres are sometimes sought abroad.

  • Biopsy report confirming squamous cell carcinoma and its grade.
  • MRI of the tongue and neck, plus CT of the chest, showing tumour depth and spread to neck nodes.
  • PET-CT, if requested, for larger tumours or suspected spread.
  • Dental assessment and X-ray, because dental work is often needed before radiotherapy.
  • Swallowing and nutrition assessment, weight records and a record of smoking, chewing tobacco and alcohol use.

  • Surgery has been advised but you have not been told what portion of the tongue will be removed and how speech and swallowing will be affected.
  • Neck dissection is recommended and you want to understand whether sentinel node biopsy or observation is possible.
  • The tumour is advanced and organ-preservation with chemoradiotherapy is being discussed.
  • A lesion or ulcer has lasted more than three weeks and has not been biopsied.

  1. What is the stage and depth of my tumour?
  2. What portion of my tongue would be removed, and what will reconstruction involve?
  3. Do I need neck surgery, and why?
  4. Will I need radiotherapy or chemotherapy afterwards?
  5. How will my speech, taste and swallowing be affected, and what rehabilitation helps?
  6. How often will I be checked, and for how long?

Outlook depends mainly on stage at diagnosis. Early, small tongue cancers treated by surgery have a good chance of cure, while outcomes fall once the tumour is deeper or has spread to neck lymph nodes. Most recurrences appear in the first two years, so close follow-up matters. Stopping tobacco and alcohol lowers the risk of recurrence and second cancers. Speech and swallowing therapy helps recovery of function after surgery.

Can tongue cancer be cured?

Yes, especially small tumours found early. The chance of cure drops as the tumour grows deeper or reaches the neck lymph nodes.

Will I be able to speak and eat after surgery?

Most people can, but function depends on the amount of tissue removed and whether reconstruction is needed. Speech and swallowing therapy are an important part of recovery.

Why is neck surgery sometimes recommended?

Tongue cancer often spreads to neck lymph nodes before it can be felt, and removing or checking them improves control of the disease.

Clinical context

Working up tongue cancer properly

A condition like tongue cancer is rarely well-served by jumping straight to treatment shopping. The sequence below is what a অনকোলজি specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or অনকোলজি specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
