PCL Tear
A posterior cruciate ligament (PCL) tear is an injury to the ligament at the back of the knee that stops the shin sliding backwards. It often follows a blow to the front of the bent knee, such as in a car accident or sports fall.

People feel pain and swelling at the back of the knee and sometimes a sense of instability when going downhill or on stairs. Examination, including the posterior drawer test, and MRI grade the tear from 1 to 3 and show injuries to other ligaments or cartilage. Isolated tears are often tolerated well, but combined injuries are less stable.
Isolated grade 1 and 2 tears are treated without surgery: protection, a hinged brace if needed, and physiotherapy to strengthen the thigh muscles. Reconstruction is considered for grade 3 tears, combined ligament injuries or persistent instability, using a tendon graft. Treatment abroad is usually considered for complex multi-ligament knee reconstruction.

Can a PCL tear heal without surgery?
Yes, often. Isolated tears, especially lower grades, can do well with bracing and physiotherapy focused on the thigh muscles. Surgery is mostly for high-grade or combined ligament injuries.
Will I develop arthritis?
There is some increased long-term risk of wear in the inner and front parts of the knee, more so with unresolved instability or combined injuries. Strength and weight control help.
Can I walk and drive normally?
Many people can, once swelling settles. Downhill walking and stairs may feel unsteady, and this is what physiotherapy targets.
How long is recovery after reconstruction?
Usually around nine to twelve months before full return to sport, because the graft and rehabilitation progress slowly.

Diagnostic workup, in sequence
- First-line clinical assessment

- Standard imaging + labs
