Dystonia
Dystonia is a movement disorder in which muscles contract involuntarily, causing twisting movements or abnormal postures. It may affect one body area (focal) or many, and it can begin in childhood or adulthood.

Examples include cervical dystonia, with the head twisting or tilting, blepharospasm, writer's cramp and generalised dystonia. Movements are often painful and may worsen with stress or activity. Diagnosis is clinical, with MRI, genetic and metabolic tests to look for causes. The course varies: some forms stay localised, while genetic or childhood-onset forms may spread.
Treatment depends on the type. Botulinum toxin injections every few months are first-line for focal forms. Oral medicines such as trihexyphenidyl, baclofen or levodopa (in dopa-responsive dystonia) help some people, along with physiotherapy. Deep brain stimulation to the globus pallidus is considered for severe generalised or medication-resistant dystonia. Specialist centres abroad may be sought for DBS.

Is dystonia curable?
Most types are not curable but can often be well controlled with botulinum toxin, medicines or deep brain stimulation. A few types respond dramatically to levodopa.
How does botulinum toxin help?
Injected into the overactive muscles, it weakens them to reduce spasm and abnormal postures. The effect wears off after about three months, so injections are repeated.
Who is suitable for deep brain stimulation?
People with severe, disabling dystonia not controlled by other treatments, particularly generalised and inherited forms, are the usual candidates. A specialist team assesses this carefully.
