Cholesteatoma

Cholesteatoma is a pocket or growth of skin cells behind the eardrum, often after long-standing ear infection or poor ear ventilation. It can erode the hearing bones and nearby structures.

Typical signs are persistent, foul-smelling ear discharge, gradual hearing loss and sometimes dizziness, ear pain or facial weakness. An ENT specialist sees the sac or retraction pocket through a microscope, and a CT or special MRI scan shows how far it extends. It does not go away by itself and tends to grow slowly, raising the risk of infection reaching the inner ear or brain.

Ear drops and cleaning can control infection for a time, but they do not remove the cholesteatoma. Surgery is the usual treatment, most often a mastoidectomy to clear the disease, sometimes with reconstruction of the eardrum and hearing bones. Follow-up scans are needed because it can return. Treatment abroad is considered when a skull base or otology surgeon with experience in complex ear disease is wanted.

  • Pure tone audiogram showing hearing level in each ear, ideally within the last few weeks.
  • High-resolution CT scan of the temporal bones and, where advised, a diffusion-weighted MRI to look for residual or recurrent disease.
  • ENT notes with otoscopy or microscope photographs, and the discharge history, including swab results.
  • Records of any previous ear surgery, with the operation notes and what was found.
  • A note of any dizziness, facial weakness or severe headaches, which should be reported before travel.

  • You have been told to observe an ear with discharge and a retraction pocket, and symptoms are continuing.
  • You have dizziness, facial weakness or a deep ear infection, which needs urgent surgical review.
  • Cholesteatoma has returned after earlier surgery, and you want a view on the best technique for this time.

  1. Where exactly is the cholesteatoma, and has it eroded bone or the hearing bones?
  2. Will the operation leave the ear canal intact or open, and what difference does that make to daily life?
  3. Can hearing be rebuilt at the same time, or in a second stage?
  4. How likely is recurrence, and what follow-up scans will I need?
  5. Which symptoms after surgery should lead to an urgent review?

Surgery removes the disease in most patients, but cholesteatoma can return, so lasting follow-up with examination and sometimes MRI is needed. Hearing may stay the same, improve or occasionally be reduced, depending on bone damage and the technique used. Left untreated, it can lead to deafness, balance problems, facial nerve damage and, rarely, brain infection.

Can cholesteatoma go away without surgery?

No. It does not resolve on its own, and ear drops only control infection. Surgery is the way to remove it, though very small or elderly, unfit patients are sometimes monitored carefully.

Can it come back after surgery?

Yes. Cholesteatoma can return or small pieces can remain, which is why follow-up examinations, a second-look operation or MRI scans are often used.

Can I fly with cholesteatoma?

Flying is usually fine, but a blocked Eustachian tube may cause pain with pressure change. If you have dizziness, facial weakness or severe ear pain, have it assessed first.

Will my hearing improve?

Not always. The first aim is a safe, dry ear. Hearing may stay the same, improve or occasionally worsen, and bone reconstruction can sometimes be done during surgery or later.

Clinical context

Working up cholesteatoma properly

A condition like cholesteatoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ইএনটি / অটোলারিঙ্গোলজি specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ইএনটি / অটোলারিঙ্গোলজি specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
