Carpal Tunnel Syndrome
Carpal tunnel syndrome is compression of the median nerve at the wrist. It causes numbness, tingling and pain in the thumb, index and middle fingers.

Symptoms are often worse at night or when holding a phone or steering wheel, and shaking the hand gives relief. Without treatment, the thumb muscles may weaken and numbness become constant. It is more common in women and in people with diabetes, pregnancy, thyroid disease or repetitive wrist work. Diagnosis is mostly clinical, with nerve conduction tests if the picture is unclear. Pregnancy-related cases often settle after birth.
Mild cases respond to a night splint holding the wrist straight, activity changes and sometimes a steroid injection. Release surgery is advised when symptoms persist, nerve tests show significant compression or there is weakness. The tight ligament is cut, either through a small open incision or endoscopically, as a day-case procedure under local anaesthetic. Most people have lasting relief of night symptoms, though numbness may recover slowly.

Carpal tunnel release relieves night pain and tingling in the great majority of people, often within days to weeks. Numbness and weakness recover more slowly and may not fully return if the nerve was compressed for a long time, which is why surgery is advised before there is muscle wasting. Recurrence is uncommon. Mild cases in pregnancy often settle spontaneously. Outcome depends on severity and duration, and on conditions such as diabetes.

Working up carpal tunnel syndrome properly
