Most patients eventually have quotes from 3+ programs. The headline numbers always look different; what matters is whether you're comparing the same scope of care.

The 12-line comparison sheet

Build a spreadsheet with these 12 rows for each program:

  1. Procedure package price (USD, locked period if relevant)
  2. Hospital stay days included
  3. Implant/device specifics (brand, size, generation)
  4. Anesthesia type and provider (anesthesiologist vs CRNA)
  5. ICU step-down availability (and per-day cost if needed)
  6. Pre-op workup acceptance (yours, or theirs?)
  7. Inpatient PT days
  8. Outpatient PT included (sessions, location)
  9. Post-discharge medications included (which, days)
  10. Follow-up imaging or labs included
  11. Telemedicine follow-up duration (months)
  12. Revision policy (window, conditions)

Most quotes don't include all 12 by default. That's the point — you're identifying scope gaps.

The "make them comparable" exercise

Once you have raw quotes, normalize them.

Step 1: Equalize hospital stay days

If Program A includes 5 days and Program B includes 7 days, ask Program A what their per-day rate is for extending. Add to A's price.

Step 2: Equalize implant brand

If A includes a Stryker Triathlon and B includes a Smith+Nephew Genesis II, ask both programs what they'd quote with the same implant. The price gap should narrow.

Step 3: Equalize PT and follow-up

Add the cost of equivalent post-op PT and follow-up to whichever program is missing it.

Step 4: Equalize anesthesiologist coverage

Both programs should specify anesthesiologist (MD) involvement. If one uses CRNA-led anesthesia, the cost may be lower but the standard isn't equivalent.

Real example: knee replacement quote comparison

ItemProgram A (India)Program B (Turkey)Program C (Thailand)
Headline package$5,500$8,500$12,500
Hospital stay included5 days4 days5 days
ImplantStrykerStrykerSmith+Nephew
Outpatient PT0 sessions10 sessions5 sessions
Telemedicine follow-up30 days90 days30 days
Revision windowImplant onlyImplant + technique 30dImplant only

Normalized for equal scope (10 PT sessions added to A and C; 90-day telemedicine added to A and C):

  • Program A normalized: $5,500 + $1,500 PT + $300 extended telemedicine = $7,300
  • Program B normalized: $8,500 (already comprehensive)
  • Program C normalized: $12,500 + $750 partial PT + $300 telemedicine = $13,550

At equalized scope, A is still cheapest by a meaningful margin — but the gap is $1,200, not $3,000.

Beyond price: quality factors

Even at equal price, programs differ on:

  • Surgeon volume on your specific procedure: ask each program for their lead surgeon's annual volume on your procedure.
  • Institutional case volume: hospitals doing 500+ knee replacements/year cluster at the lower end of complication rates.
  • Communication quality: response time during the quote phase predicts response time during your trip.
  • Transparent outcome data: programs that publish revision rates and complication rates are typically the strongest.

What I'd actually do

For a moderate-complexity procedure:

  1. Get 3–4 quotes.
  2. Normalize using the 12-line sheet.
  3. Eliminate the cheapest if it's missing critical scope (PT, follow-up, ICU).
  4. Eliminate the most expensive if the premium isn't justified by superior outcomes data.
  5. Choose between the middle two based on surgeon volume + communication quality.

Don't pick on price alone. The cost of the wrong program (revision, complication, return trip) dwarfs the cost difference between top-tier programs.
